Horizon Healthcare Medical Supplies, LLC aka HorizonHCS accepts Visa®, MasterCard® and American Express®.  Net 30-day terms are available to accounts upon credit approval.  HorizonHCS reserves the right to deny purchase at any time.

Returns, Exchanges & Warranty Repairs

All returns, exchanges and warranty repairs must have prior authorization or credit will not be issued or repair work will not be completed.  Call 1-866-740-2110 for authorization, return address and procedures.  Pre-authorized returns must be returned within 30-days of the invoice date or restocking fees may apply.  All items must be returned in their original packaging and in resaleable condition.  Personal use and custom items are non-returnable.  Credit will only be issued after a thorough inspection is completed and is subject to credit on account only.  Credits are valid for one year from the date of issuance.  HorizonHCS assumes no responsibility for any merchandise that is lost or damaged during return shipment.  On customer arranged returns/repairs, it is the customers responsibility to ensure that items are returned to their proper destination and are responsible for keeping tracking numbers and proof of delivery if needed.  On return shipments via call tag, it is the customers responsibility the have the item(s) packaged with the correct return address within 24 hours of receiving the return authorization.  The package must also be placed in an accessible location for pick-up by the carrier.  HorizonHCS recommends that on large orders, one item be ordered in advance to ensure it meets the requirements of the buyer, as returns on large orders may be subject to warehouse restocking fees.  All warranties are provided by the manufacturer unless specified otherwise.

Receiving Shipments

Inspect all packages thoroughly before signing for receipt of goods.  If packages are in any way damaged, you must either immediately inspect the goods or note the damage on the receipt prior to signing.  Signing for a shipment without documenting damaged containers/boxes releases the shipper of all responsibility and liability.

Taxes

Horizon Healthcare Medical Supplies, LLC now utilizes Alavara to collect and remit sales tax for all states and the District of Columbia, with the exception of these states: AK, DE, MT, NH & OR.  If a customer is tax exempt, It is their responsibility to ensure that Horizon Healthcare Medical Supplies, LLC has the correct tax exempt forms on file or sale tax will be charged.
